Course Aims

Incident Cause Analysis Method (ICAM) is an Incident Investigation methodology drawing on research on Human Error and its contribution to incidents by Professor James T Reason.

 

Our 1-day Incident Investigation course is based on Incident Cause Analysis Method, and provides a structured methodology used for incident investigation and focuses on identifying root causes and contributing factors by analysing human, organizational, and technical failures. It is primarily for lead investigators but can be delivered to Team Members that are expected to be part of an incident investigation team to gain an applied understanding of the investigation methodology and learn the knowledge and skills required to be an effective team member in incident investigations. 

The course is aimed at improving organizational safety culture by addressing root causes and preventing recurrence of incidents. Participants gain a thorough understanding of the ICAM methodology and are able to apply it to conduct detailed and effective investigations.

   

Who Should Do the Course?

The course is designed for individuals who will lead incident investigations or play a significant role in analysing complex incidents, and personnel who may assist the Lead Investigator in collecting data and conducting low-level incident investigations.

  • Identifying and collecting forms of evidence for the investigation

  • Interviewing witnesses

  • Identifying causative factors for accidents and incidents using accident causation models

  • Communicating the outcomes of investigation
